When a facility evaluates alternating pressure or low air loss mattresses, the comparison is almost always unit price. That is the number in the quote, the number in the budget line, and the number the ownership group asks about.
It is also the smallest number involved.
Follow one failure through the building
A pump alarms on a Tuesday night. Here is what that single event actually consumes, department by department. None of it is resident care. All of it is paid time.

Most facilities that work through this exercise land somewhere around two to two and a half staff-hours per swap event. Your building may be faster or slower depending on how far the storage room is and whether a spare is available at all.
The part that does not get counted: alarms that never end in a swap. Every silenced and reset nuisance alarm still pulls a nurse or aide to the bedside for five to ten minutes — on every shift, including nights, when staffing is thinnest and a resident being woken has consequences beyond the equipment.
The arithmetic across a portfolio
Take a building with four swap events in a month. That is roughly eight to ten staff-hours — more than a full shift — spent moving mattresses instead of caring for residents. Across a multi-building portfolio, that compounds into hundreds of hours a year.
And the hours are the visible cost. The rest is harder to price:
- Night-shift alarm fatigue. Staff who stop responding urgently to a nuisance alarm are being trained by the equipment to ignore alarms generally.
- Resident sleep disruption. A resident on a therapeutic surface is there because they are at risk. Waking them repeatedly is not neutral.
- Family perception. Equipment that visibly fails and gets swapped is noticed by visitors, and it becomes part of how they describe the building to others.
- Survey exposure. Equipment maintenance records and preparedness plans are inspected. A fleet with a known failure pattern and no service documentation is a weak position.
Where the cost actually originates
It is worth being precise about causes, because the fix depends on which one you have.
- Pumps with no airflow reserve. Low air loss bleeds air continuously. A pump with little margin above that bleed will fall behind whenever anything is slightly imperfect. We wrote about the mechanics of this separately.
- Manual weight settings. Every placement depends on a person selecting correctly. Pumps that sense the resident and set themselves remove the error rather than training around it.
- Materials that degrade. Cells that harden under routine disinfection and base covers that thin where they drag across the deck produce failures that look random but are not.
- Maintenance that never happens. Air filters are the single highest-yield service task on a mattress fleet, and in most buildings nobody owns them.
A more useful way to compare vendors
If unit price is the only column on the scorecard, the cheapest system wins every time and the labor lands on your staff. A more honest comparison includes:
- Pump airflow, and whether it has reserve under continuous low air loss
- Whether staff must configure anything at placement
- Documented disinfectant compatibility for the agents housekeeping actually uses
- Who performs swap-outs, cleaning of the returned unit, and warranty paperwork — you or the vendor
- Whether filter service happens on a schedule, and whose schedule
